“Eles identificaram que 41.6% dos pacientes avaliados apresentavam doen\u00e7a de superf\u00edcie ocular, sendo que o risco aumentava de acordo com o maior n\u00famero de medica\u00e7\u00f5es t\u00f3picas, maior n\u00famero de instila\u00e7\u00f5es di\u00e1rias, uso prolongado, maior exposi\u00e7\u00e3o a col\u00edrios com BAK.”<\/span>O glaucoma e a s\u00edndrome da disfun\u00e7\u00e3o lacrimal (doen\u00e7a da superf\u00edcie ocular) s\u00e3o patologias oculares que tem sua preval\u00eancia aumentada com o envelhecimento. N\u00e3o \u00e9 rara a associa\u00e7\u00e3o destas duas patologias.<\/p>\n

Outro fator agravante \u00e9 que os pacientes portadores de glaucoma necessitam do uso cr\u00f4nico de col\u00edrios, muitas vezes para o resto da vida. Este uso cr\u00f4nico pode levar ao aparecimento e\/ou agravamento desta doen\u00e7a da superf\u00edcie ocular.<\/p>\n

Acredita-se que um dos principais respons\u00e1veis pelo aumento da preval\u00eancia de doen\u00e7a da superf\u00edcie ocular nos pacientes com glaucoma \u00e9 a presen\u00e7a de conservantes como BAK na f\u00f3rmula dos col\u00edrios.<\/p>\n

“entretanto, para uma correta interpreta\u00e7\u00e3o e aplicabilidade no consult\u00f3rio, e importante ressaltar que mais da metade dos pacientes em tratamento clinico nao apresentavam doen\u00e7a de superf\u00edcie ocular.”Os autores deste estudo transversal tentaram elucidar quais eram os fatores associados \u00e0 presen\u00e7a de doen\u00e7a da superf\u00edcie ocular<\/span> em pacientes em tratamento de glaucoma.<\/p>\n

Eles identificaram que 41.6% dos pacientes avaliados apresentavam doen\u00e7a de superf\u00edcie ocular, sendo que o risco aumentava de acordo com o maior n\u00famero de medica\u00e7\u00f5es t\u00f3picas, maior n\u00famero de instila\u00e7\u00f5es di\u00e1rias, uso prolongado, maior exposi\u00e7\u00e3o a col\u00edrios com BAK. Os pacientes com estes fatores apresentavam maior chance de apresentar doen\u00e7a da superf\u00edcie ocular e uma pior qualidade de vida.<\/span><\/p>\n

Os autores concluem que para evitar o aparecimento e\/ou agravamento da doen\u00e7a da superf\u00edcie ocular em pacientes tratados para glaucoma deve-se procurar diminuir a exposi\u00e7\u00e3o ao BAK e diminuir o n\u00famero de medica\u00e7\u00f5es\/instila\u00e7\u00f5es di\u00e1rias (atrav\u00e9s do uso de combina\u00e7\u00f5es fixas, por ex.), entretanto, para uma correta interpreta\u00e7\u00e3o e aplicabilidade no consult\u00f3rio, e importante ressaltar que mais da metade dos pacientes em tratamento clinico nao apresentavam doen\u00e7a de superf\u00edcie ocular.<\/p>\n

\n Abstract<\/strong><\/p>\n

PURPOSE<\/strong>:
\nTo identify risk factors for developing ocular surface disease (OSD), to verify the prevalence of OSD, and to record efficacy of questionnaires in identifying symptoms’ impact on patients’ quality of life.<\/p>\n

METHODS<\/strong>:
\nThis was an observational, cross-sectional study of patients with topically treated glaucoma. Tear film break-up time (TFBUT) and punctate keratitis were evaluated; 2 quality-of-life questionnaires (National Eye Institute-Visual Function Questionnaire 25 and Glaucoma Symptom Scale) were submitted to all patients. Class of previous and current intraocular pressure (IOP)-lowering drugs, number of drugs, number of drops\/day, and total and current benzalkonium chloride (BAK) exposure were collected.<\/p>\n

RESULTS<\/strong>:
\nA total of 233 patients completed the study. TFBUT was abnormal in 71 (30.5%) eyes; punctate keratitis was present in 74 (31.7%). Keratitis was more frequent with increasing number of eyedrops (p=0.008) and number of instillations per day (p=0.009). Ocular surface disease was present in 97 (41.6%) patients and was statistically related to number of medications used (p=0.026). The univariate analysis pointed out that patients with OSD were older (p=0.04), had lower IOP values (p=0.03), were topically treated for more time (p<0.0001), had assumed more BAK (p<0.0001), and presented worst quality of life (p<0.01). The multivariate analysis found that OSD was related to number of medications used (p=0.002), prolonged use of preserved medications (p=0.005), and total BAK exposure (p<0.001).<\/p>\n

CONCLUSIONS<\/strong>:
\nThere is clinical evidence that the number of medications, their prolonged use, and the total BAK exposure are risk factors to develop OSD in patients with glaucoma. To prevent OSD onset, BAK exposure and the number of topical medications should be reduced.<\/p>\n
