Com aumento da resolu\u00e7\u00e3o e da velocidade de aquisi\u00e7\u00e3o de imagem, \u00e9 poss\u00edvel medir diversos par\u00e2metros estruturais como a espessura da camada de fibras nervosas da retina (CFNR), topografia do disco \u00f3ptico e an\u00e1lise segmentar da regi\u00e3o macular (GCC; ganglion cell complex).<\/p>\n<p>Diversos estudos pr\u00e9vios avaliaram e validaram esses diferentes protocolos para o diagn\u00f3stico de glaucoma, no entanto, a popula\u00e7\u00e3o estudada era composta de indiv\u00edduos com glaucoma confirmado por defeito de campo visual versus pacientes normais sem suspeita cl\u00ednica de glaucoma (hipernormal?). <strong>No cen\u00e1rio cl\u00ednico real, o oftalmologista solicita o exame de SD-OCT para auxiliar no diagn\u00f3stico de paciente sem doen\u00e7a confirmada por outros testes.<\/strong> A maioria dos estudos anteriores que avaliaram a acur\u00e1cia do RTVue SD-OCT (Optovue Inc.) para detectar glaucoma, na realidade, mediram a capacidade para diferenciar pacientes com defeitos campim\u00e9tricos confirmados e pacientes sem sinais de glaucoma, o que, obviamente, n\u00e3o \u00e9 representativo da popula\u00e7\u00e3o clinicamente relevante.<\/p>\n<p><strong>Para avaliar os diferentes protocolos dispon\u00edveis no RTvue em um cen\u00e1rio mais pr\u00f3ximo do clinicamente relevante, Lisboa e colaboradores publicaram um estudo longitudinal<\/strong><\/p>\n<p><strong>avaliando 142 olhos de pacientes suspeitos de terem glaucoma pela apar\u00eancia do disco \u00f3ptico e com campos visuais normais. Ap\u00f3s cerca de 13 anos de seguimento, esses olhos foram classificados em dois grupos de acordo com a presen\u00e7a ou aus\u00eancia de progress\u00e3o estrutural observadas \u00e0 retinografia. O grupo que apresentou progress\u00e3o no disco \u00f3ptico ao longo do tempo foi classificado como glaucoma enquanto que o grupo sem progress\u00e3o longitudinal foi usado como controle. Todos apresentaram campos visuais normais ao longo de todo o acompanhamento.<\/strong><\/p>\n<p>Os resultados apresentados demostram uma performance diagnostica pior do que as encontradas em estudos anteriores publicados na literatura para o mesmo aparelho, indicando a import\u00e2ncia da sele\u00e7\u00e3o da popula\u00e7\u00e3o nos estudos sobre acur\u00e1cia diagn\u00f3stica. Entretanto, esses resultados aproximam-se mais da realidade na qual este aparelho e utilizado na pratica clinica di\u00e1ria \u2013 pacientes com suspeita de glaucoma. E dentro deste contexto, o SD-OCT foi capaz de detectar pacientes com glaucoma pre-perimetrico.<\/p>\n<p><span class=\"destaqueBgVerdeFloat\"> Os resultados apresentados demostram uma performance diagnostica pior do que as encontradas em estudos anteriores publicados na literatura para o mesmo aparelho, indicando a import\u00e2ncia da sele\u00e7\u00e3o da popula\u00e7\u00e3o nos estudos sobre acur\u00e1cia diagn\u00f3stica. Entretanto, esses resultados aproximam-se mais da realidade na qual este aparelho e utilizado na pratica clinica di\u00e1ria \u2013 pacientes com suspeita de glaucoma. E dentro deste contexto, o SD-OCT foi capaz de detectar pacientes com glaucoma pre-perimetrico.<\/span><\/p>\n<p>Apesar de outros estudos na literatura mostrarem equival\u00eancia ou at\u00e9 superioridade do par\u00e2metro do GCC sobre a espessura da CFNR, <strong>os autores reportaram maior performance diagn\u00f3stica dos par\u00e2metros de CFNR sobre GCC e topografia do disco \u00f3ptico. <\/strong>Os par\u00e2metros de melhor performance para cada protocolo foram: M\u00e9dia da espessura da CFNR (\u00e1rea sob curva ROC de 0.89 \u00b1 0.03, com sensibilidade de 71% sob especificidade de 95%), raz\u00e3o escava\u00e7\u00e3o\/disco na topografia do disco \u00f3ptico (\u00e1rea sob curva ROC de 0.74 \u00b1 0.04, com sensibilidade de 19% sob especificidade de 95%) e m\u00e9dia da espessura para o GCC (\u00e1rea sob curva ROC de 0.79 \u00b1 0.04, com sensibilidade de 44% sob especificidade de 95%). \u00c9 importante notar a avalia\u00e7\u00e3o da topografia do disco \u00f3ptico apresenta desvantagem em estudos que levam em considera\u00e7\u00e3o o aspecto do disco \u00f3ptico como crit\u00e9rio de inclus\u00e3o.<\/p>\n<p><strong>Esse estudo mostra que o RTVue foi capaz de diferenciar indiv\u00edduos com glaucoma pr\u00e9-perim\u00e9trico dos pacientes com discos suspeitos sem progress\u00e3o estrutural ao longo do tempo. All eyes had normal visual fields before the imaging session. Forty-eight eyes with progressive glaucomatous damage were included in the preperimetric glaucoma group. Ninety-four eyes without any evidence of progressive glaucomatous damage and followed untreated for 12.8 \u00b1 3.6 years were used as controls. Areas under the receiver operating characteristic curves (AUC) were calculated to summarize diagnostic accuracies of the parameters.<\/p>\n<p><strong>RESULTS:<\/strong><\/p>\n<p>The three RNFL parameters with the largest AUCs were average RNFL thickness (0.89 \u00b1 0.03), inferior hemisphere average thickness (0.87 \u00b1 0.03), and inferior quadrant average thickness (0.85 \u00b1 0.03). The three ONH parameters with the largest AUCs were vertical cup-to-disc ratio (0.74 \u00b1 0.04), rim area (0.72 \u00b1 0.05), and rim volume (0.72 \u00b1 0.05). The three macular parameters with the largest AUCs were GCC average thickness (0.79 \u00b1 0.04), GCC inferior thickness (0.79 \u00b1 0.05), and GCC superior thickness (0.76 \u00b1 0.05). Average RNFL thickness performed better than vertical cup-to-disc ratio (0.89 vs. 0.74; P = 0.007) and GCC average thickness (0.89 vs. 0.79; P = 0.015).<\/p>\n<p><strong>CONCLUSIONS:<\/strong><\/p>\n<p>SDOCT RNFL measurements performed better than ONH and macular measurements for detecting preperimetric glaucomatous damage in a cohort of glaucoma suspects.
