Na maioria desses estudos, os pacientes s\u00e3o previamente classificados em normais ou portadores de glaucoma em uma an\u00e1lise transversal. Em seguida, o exame de imagem \u00e9 realizado e a habilidade de diferenciar os dois grupos \u00e9 medida.O problema \u00e9 que, em quase todos os estudos, o grupo dos portadores de glaucoma \u00e9 definido por indiv\u00edduos que j\u00e1 apresentam defeitos no campo visual, e o grupo controle \u00e9 composto de indiv\u00edduos que n\u00e3o apresentam sinais de glaucoma (PIO, campo visual e apar\u00eancia do disco optico normais)<strong>, <\/strong>possivelmente hiperestimando a acur\u00e1cia diagnostica.<\/p>\n<p><span class=\"destaqueBgVerdeFloat\">O problema \u00e9 que, em quase todos os estudos, o grupo dos portadores de glaucoma \u00e9 definido por indiv\u00edduos que j\u00e1 apresentam defeitos no campo visual, e o grupo controle \u00e9 composto de indiv\u00edduos que n\u00e3o apresentam sinais de glaucoma (PIO, campo visual e apar\u00eancia do disco optico normais)<strong>, <\/strong>possivelmente hiperestimando a acur\u00e1cia diagnostica.<\/span><\/p>\n<p><strong>Para contornar essas importantes limita\u00e7\u00f5es e buscando avaliar a utilidade dos exames de imagem na pr\u00e1tica, Lisboa e colaboradores seguiram longitudinalmente 134 olhos suspeitos de glaucoma baseando-se na apar\u00eancia suspeita do disco \u00f3ptico, todos com campos visuais normais ao longo de todo acompanhamento. Ap\u00f3s um seguimento m\u00e9dio de cerca de 14 anos,<\/strong> <strong>os pacientes foram classificados em dois grupos de acordo com a presen\u00e7a ou aus\u00eancia de progress\u00e3o estrutural observadas \u00e0 retinografia.<\/strong><\/p>\n<p>O grupo que apresentou progress\u00e3o no disco \u00f3ptico ao longo do tempo foi classificado como glaucoma e os pacientes sem altera\u00e7\u00e3o no disco optico ao longo do tempo foram usados como controle. Os autores ent\u00e3o avaliaram a acur\u00e1cia diagnostica do Spectralis SD-OCT (Spectral Domain Otic Coherence Tomography, Heidelberg Engineering) e do HRT (Heidelberg retina tomograph, Heidelberg Engineering) em detectar os casos de glaucoma pr\u00e9-perim\u00e9trico de pacientes com disco optico suspeito mas sem progress\u00e3o ao longo de 14 anos (media), ou seja, nos casos duvidosos que realmente interessam na pratica clinica di\u00e1ria.<\/p>\n<p>O artigo apresenta diversos resultados interessantes com aplica\u00e7\u00f5es pr\u00e1ticas. <strong>A compara\u00e7\u00e3o dos melhores par\u00e2metros de cada aparelho mostrou superioridade estatisticamente significante do Spectralis SD-OCT <\/strong>(area sob curva ROC = 0.88 \u00b1 0.03, com a melhor sensibilidade de 48% sob especificidade de 95%), <strong>comparado ao HRT <\/strong>(area sob curva ROC = 0.72 \u00b1 0.04, com a melhor sensibilidade de 25% sob especificidade de 95%)<strong>, indicando que, no grupo estudado, a an\u00e1lise da espessura da camada de fibras nervosas da retina foi superior \u00e0 an\u00e1lise da topografia do disco \u00f3ptico. <\/strong>Os autores tamb\u00e9m inclu\u00edram uma an\u00e1lise dos likelihood ratios, que levando em considera\u00e7\u00e3o a classifica\u00e7\u00e3o normativa do instrumento e fornece informa\u00e7\u00f5es sobre a probabilidade de um exame alterado indicar doen\u00e7a e um resultado normal indicar aus\u00eancia de doen\u00e7a. Eles mostraram que a probabilidade de um teste classificado como anormal indicar glaucoma \u00e9 maior no Spectralis do que no HRT. All eyes underwent retinal nerve fiber layer (RNFL) imaging with Spectralis SD-OCT (Heidelberg Engineering, Carlsbad, CA) and topographic imaging with Heidelberg Retinal Tomograph III (HRT-III) (Heidelberg Engineering) CSLO within 6 months of each other. All patients had normal visual fields at the time of imaging and were classified on the basis of history of documented stereophotographic evidence of progressive glaucomatous change in the appearance of the optic nerve occurring before the imaging sessions.<\/p>\n<p><strong>MAIN OUTCOME MEASURES:<\/strong><\/p>\n<p>Areas under the receiver operating characteristic curves (AUCs) were calculated to summarize diagnostic accuracies of the SD-OCT and CSLO. Likelihood ratios (LRs) were reported using the diagnostic categorization provided by each instrument after comparison to its normative database.<\/p>\n<p><strong>RESULTS:<\/strong><\/p>\n<p>Forty-eight eyes of 42 patients had evidence of progressive glaucomatous change and were included in the preperimetric glaucoma group. Eighty-six eyes of 46 patients without any evidence of progressive glaucomatous change followed untreated for an average of 14.0 \u00b1 3.6 years were included in the control group. The parameter with the largest AUC obtained with the SD-OCT was the temporal superior RNFL thickness (0.88 \u00b1 0.03), followed by global RNFL thickness (0.86 \u00b1 0.03) and temporal inferior RNFL thickness (0.81 \u00b1 0.04). The parameter with the largest AUC obtained with the CSLO was rim area (0.72 \u00b1 0.05), followed by rim volume (0.71 \u00b1 0.05) and linear cup-to-disk ratio (0.66 \u00b1 0.05). Temporal superior RNFL average thickness measured by SD-OCT performed significantly better than rim area measurements from CSLO (0.88 vs. 0.72; P=0.008). Outside normal limits results for SD-OCT parameters were associated with strongly positive LRs.<\/p>\n<p><strong>CONCLUSIONS:<\/strong><\/p>\n<p>The RNFL assessment with SD-OCT performed well in detecting preperimetric glaucomatous damage in a cohort of glaucoma suspects and had a better performance than CSLO.<\/p>\n<p>Copyright \u00a9 2012 American Academy of Ophthalmology.
